I/We understand and agree that:

Admission to the program is based on availability and thus it is not guaranteed unless confirmed by the HPS Director;
Payment in full must be made electronically in order to register my child for the summer program;
There are no refunds once payment has been submitted;
A $50 application fee is mandatory and non-refundable;
Due to staffing and enrollment constraints, it is not possible to change my schedule such as exchange one week for another or one day for another;
Additional fees may apply if I am late picking up my child(ren);[1]
If my child’s continued attendance at the school is deemed by the Director to be detrimental to his or her well being, or the well being of other students, my child(ren) may be suspended or dismissed from the program. Full refund will be made if such an event takes place. There will be no refund if I make the decision to withdraw my child for any reason from any of the sessions in which he/she is enrolled.
I understand that acceptance to the HPS Summer Camp depends on providing the director with the following: up to date DC Health Certificate and Oral Health Form, and all updated immunization records (including Covid-19), TB and Lead tests; Emergency Information Form; Travel and Activity Authorization Form; Authorization for Emergency Medical Treatment Form; Registration for Child Receiving Care Away from Home Form; Photographic Authorization Form; Medication Authorization Form. I further understand that all forms must be completed and provided to the HPS office prior to camp beginning in order for this contract to be in effect.
A sleeping mat is mandatory; a quiet time is offered to our campers most days;
My child must wear closed toe shoes when attending summer camp. Sending my child to school wearing clogs, flip flops or jelly shoes will result in sending my child home to change;
Families are expected to follow all Covid policies and procedures as outlined on the school website and parent handbook. All campers are required to be fully vaccinated against Covid-19.
